This app also provides information about hospice eligibility, as well as a convenient way for healthcare providers to refer patients to Hospice Circle of Love. Referrals submitted through the app will be received by the Circle of Loves admissions office. All information received through the referral form complies with and will be processed according to HIPAA regulations.
